Job Description

  • Ensure the psycho-social and emotional well-being of the residents are well taken care of, so as to enable them to cope with their independent living and to enhance their overall quality of life

  • Provide person-centric care through the Social Work Code of Professional Ethics, interprofessional collaboration practices and practising holistic care philosophy to enhance overall service delivery to Home residents.

  • Respond to enquiries from stakeholders and the public within 3 working days.

  • Respond to the assigned referrals by contacting the referral party within 3 working days to arrange pre-admission assessments.

  • Orientate newly admitted residents, which would commence on next day of admission.

  • Conduct assessments and develop Individualised Care Plans (ICP) based on the bio-psycho-social-spiritual (BPSS) model and Holistic Care Philosophy for new residents within two months of admission, and every 6 months upon admission for assigned residents.

  • Ensure at least 70% of assigned residents attained success in their functional goals based on their ICP.

  • Responsible for case management, which includes coordination of services and resources, monitoring of residents’ physical/emotional/social well-being and coping, co-working with residents towards achieving their goals, and discharge planning.

  • Plan and implement group work for residents to build coping abilities, promote mental/emotional well-being and improve their quality of life.

  • Ensure accurate and timely documentation of case notes, incidents, discharge plans and/or records pertaining to assigned residents.

  • Manage disputes by mediating conflicts between residents, facilitating communication, maintaining accurate documentation of conflicts and intervention, and working towards peaceful resolutions among residents.

  • Provide regular updates to residents’ next-of-kin and/or caregivers with the aim of building closer ties between residents and their loved ones for the assigned residents.

  • Conduct financial-related applications and assessments, such as means-testing, financial assistance scheme, subsidy deviation, and Medical Fee Exemption Card (MFEC), for assigned residents.

  • Collate and prepare relevant client data and information for monthly billing, quarterly/biannual reports and yearly/ad-hoc audits by relevant authorities.

  • Protect residents’ confidentiality by adhering strictly to organisational Personal Data Protection Policy and national Personal Protection Data Act (PDPA) and regulations.

  • Adhere to organisational and service Standard Operating Procedures and service requirements at all times.

  • Participate in multi-disciplinary team meetings and work with the Nursing, Operations and Programmes teams in formulating ICPs for all residents.

  • Part of the HSC team of Social Workers to seek out continuous learning opportunities and share best practice knowledge with others to enhance overall service delivery to residents.


Requirements

  • Degree or Graduate Diploma in Social Work

  • Registered or eligible for registration with the Singapore Association of Social Workers (SASW)

  • Knowledge of the Social Work Code of Professional Ethics, case management frameworks, and relevant legislation (e.g., PDPA)

  • At least 2 to 4 years of experience practicing social work in community care sector or residential settings is preferred

  • Understanding of the geriatric landscape in Singapore

  • Competent in BPSS assessment, case management, and intervention planning (including mitigating risks at client’s level) & practice evaluation

  • Excellent communication and counselling skills with elderly clients, their caregivers and with multidisciplinary professionals.

  • Ability to maintain confidentiality and professional boundaries in line with SASW Code of Ethics

  • Ability to draw and apply relevant theories in direct case work for appropriate intervention/ care plans

  • Ability to identify, profile and prioritize clients based on needs evaluation

  • Proficient in MS Word, Excel and PowerPoint and capable of maintaining clear and timely case documentation and reports.

  • Fluent in English and conversational proficiency in mother tongue
