As a Senior Executive, Primary Care Engagement & Transformation (Healthcare), you will be responsible for the following duties:

• Implement initiatives to strengthen partnerships with GPs and community healthcare providers for accessible and affordable care.

• Maintain relationships with primary care partners to understand their needs and address community health priorities.

• Coordinate resources and support for GPs to enhance their ability to manage patient care and resolve operational issues.

• Maintain and update the database of primary care partners to ensure smooth collaboration and follow-up.

• Execute shared care and community programmes to enable seamless patient transitions between primary and tertiary care.

• Facilitate access for primary care partners to relevant services, such as direct referral and shared care arrangements.

• Support quality improvement initiatives aligned with Healthier SG preventive care goals.

• Facilitate knowledge sharing, training sessions, and best-practice exchanges for primary care partners.

• Monitor and review programmes to identify opportunities for process improvement and resource optimisation.

Requirements:

• Degree holder in any related discipline.

• At least 2-3 years working experience in community care/ marketing/ business, preferably in healthcare setting.
