About the role

We are seeking an experienced and compassionate Social Worker to join our team at the Sree Narayana Mission (Singapore) nursing home in the Yishun North Region. In this full-time position, you will play a vital role in supporting the holistic well-being of our elderly residents, providing personalised care and social services to enhance their quality of life.

What you'll be doing

1. Conduct comprehensive assessments to understand the individual needs, challenges and concerns of each resident

2. Develop and implement tailored care plans to address physical, emotional, social and practical requirements

3. Facilitate group and individual counselling sessions to provide emotional support and promote social engagement

4. Collaborate with the multidisciplinary care team to ensure a cohesive approach to resident care

5. Advocate for residents' rights and needs, liaising with families, healthcare providers and community resources

6. Maintain detailed case notes and documentation to monitor progress and inform care strategies

7. Participate in quality improvement initiatives and contribute to the continuous enhancement of service delivery

What we're looking for

1. Bachelor's degree in Social Work or a related field

2. Minimum 3 years of experience as a Social Worker, preferably in a long-term care or healthcare setting

3. Strong assessment, counselling and case management skills

4. Excellent communication and interpersonal abilities, with a compassionate and empathetic approach

5. Proficiency in managing sensitive information and adherence to confidentiality protocols

6. Ability to work collaboratively within a multidisciplinary team

7. Registration with the Singapore Association of Social Workers (SASW) is preferred
