Summary:

• Location: Jurong West/Tengah Region

• Working Hours: Office Hourse

Responsibilities:

• Build rapport with residents/clients and their families. Maintain constant communication with clients while addressing their concerns and goals and helping them keep positive attitudes.

• Provide casework management for our clients and coordination with external agencies such as hospitals and polyclinic for clients who require financial assistance for their medical appointments.

• Develop and implement individual care plans (ICP) for clients and provide therapeutic support as needed.

• Execute the ICP and monitor progress. Develop and implement programmes and activities through a planned/structured manner.

• Financial assessment and fund management and prepare periodic reports as required - identifying and referring cases for financial assistance.
