Responsibilities:

• Participate in daily interdisciplinary rounds to develop and implement patient care plans in a home-based hospital setting
• Conduct virtual ward rounds to ensure optimal pharmacological care and patient outcomes
• Manage complex inpatient cases across various specialties using clinical expertise
• Optimize medication regimens to improve adherence and minimize adverse effects
• Provide expert consultation on medication safety and efficacy
• Educate patients to support effective home-based therapy management
• Contribute to clinical governance, strategic planning, and protocol development
• Deliver general clinical pharmacy services including medication reconciliation and adherence monitoring
• Identify and manage patient-specific pharmacotherapy issues
• Monitor and adjust drug therapies in collaboration with healthcare providers
• Promote adherence to prescribing guidelines and drug use protocols
• Provide evidence-based drug information to patients and healthcare teams
• Ensure safe, cost-effective, and timely medication distribution
• Communicate with patients and caregivers to resolve drug-related issues and support therapy compliance
• Document clinical activities according to department guidelines
• Lead or participate in clinical practice committees
• Plan and deliver educational programs for healthcare professionals and the public
• Develop patient education materials and contribute to professional publications
• Lead competency framework development for future clinical pharmacists
• Contribute to continuing professional development strategies
• Initiate and participate in clinical research and publish findings
• Present research at local or international scientific meetings
• Lead service evaluations and implement medication safety improvements
• Undertake departmental or institutional projects aligned with organizational goals
• Maintain an up-to-date professional portfolio

Requirements:

• MSc (Clinical Pharmacy), Pharm D, or Board Certification is required
• Completion of a Collaborative Prescribing Course within the last 2 years
• Minimum 5 years of clinical experience, preferably in inpatient care
• Registered with the Singapore Pharmacy Council
• Strong leadership, supervisory, and managerial skills
• Proficient in clinical problem-solving and decision-making
• Strong communication and patient education skills
