Job Responsibilities:

• Initiate contact with adolescent patients visiting the clinic.

• Accompany the Medical Social Worker on field visits as needed.

• Manage email and phone inquiries related to the adolescent mental health program.

• Maintain program data.

• Schedule and coordinate meetings.

• Assist the Medical Social Worker with case management tasks and other patient profiles/projects as required.

Qualifications

• Diploma in Social Work or a related field.

• At least 1 year of relevant working experience in social services or healthcare settings.

• Strong interpersonal and communication skills, with empathy and professionalism.
