(Senior) Assistant Manager , Care/Community Partnership (Healthcare) - #1100737

Key Responsibilities

Community Engagement & Development

• Build trust with community residents and stakeholders to identify community strengths, support networks, and shared priorities.

• Conduct placed-based outreach (1:1 conversation) using both lived experience and data to understand individual/community aspiration

• Facilitate community-led efforts for social connection and wellbeing

• Support residents in navigating health and social systems to enable independence and long-term resilience

Partnership Development

• Maintain partnerships with internal teams/external agencies for coordinated care/solutions

• Facilitate organisation-community info-sharing to close care gaps

Capability Building

• Coach teams on strengths-based, community-first approaches

• Support training for asset-based care planning and service design

• Develop evaluation metrics that reflect individual outcomes and community contributions to care

Transitional Care Integration

• Develop community-anchored discharged plans to cater for patients with complex needs

• Connect residents to local resources that reduce readmissions and support home/community recovery

• Monitor outcomes/improve post-discharge care continuity

Administrative

• Prepare periodic reports to update the management

• Any other projects/tasks assigned

Job Requirements

• Bachelor’s degree in community development, public health, social work or related field

• 3-5 years of work experience in population health, care integration or community development settings

• Familiarity with strengths-based approaches and participatory methods

• Competence in programme evaluation and basic data analysis
