Nurse Counsellor (Office Hours/ Work in GP/5 days) (ID:678008) - #1099820

• Provide structured nurse counselling, health screenings, patient education on chronic diseases, mental health management, and health promotion through in-person or virtual consultations with a network of GP.

• Identify patients for nurse counselling to improve their understanding of chronic conditions, social history, lifestyle, and dietary habits, enhancing overall health outcomes.

• Collaborate with GPs to create personalized care plans for managing chronic conditions and educate patients on administering insulin, monitoring glucose, managing blood pressure, using inhalers, and following diet plans.

• Conduct health screenings, including eye and foot exams for diabetic patients, spirometry for asthma or COPD diagnosis, and assess mental health, escalating concerns to GPs when needed.

• Assist with tracking clinical outcomes, scheduling ancillary services, supporting quality improvement projects, and performing additional duties as directed by the Reporting Officer.

Job Requirements

• Degree in Nursing with Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management will be an advantage 

• Registered Nurse with the Singapore Nursing Board

• Excellent communication and interpersonal skills are essential

• Willing to travel and work on Saturdays, when required.
