Responsibilities:

• Support the care team in coordinating and improving the patient’s care
• Work closely with other nursing and allied team to provide advocate care options and services to the patients and their family members
• Improve the patient’s care plan by understanding their needs
• Involve in community outreach programmes

Requirements:

• Degree in Social Work/Psychology
• Working experience in healthcare or social service sector is an advantage
• Ability to multi-task and work in a fast-paced and dynamic environment
