Responsibilities

• Conduct health screenings and basic patient assessments.
• Educate patients on chronic disease management, healthy lifestyles and preventive care.
• Perform blood pressure, glucose, vision, mental health and other basic assessments.
• Arrange referrals, appointments, follow-ups and investigations.
• Coordinate patient care with the healthcare team.
• Provide telephone follow-up and patient support.
• Maintain accurate nursing documentation and clinical records.
• Follow patient safety, infection control and clinic policies.
• Participate in health promotion and quality improvement activities.
• Attend training to maintain professional knowledge and skills.
• Maintain clinic cleanliness, equipment and supplies.
• Perform other duties assigned by the supervisor.

Requirement :

• Polytechnic Diploma in Health Management Services and Health Promotion
• Good communication and interpersonal skills.
• Able to work independently and as part of a healthcare team.
