Part-time Youth Social Worker/Case Manager, Student Outreach (Primary School) - #1165450

This position will start off as 1 year contract.

Duties:

1) Outreach and Engagement

The officer conducts home visits and outreach to SwANs (Students with Additional Needs) with prolonged or irregular attendance, establishing rapport with students and their parents or caregivers to better understand barriers to school attendance. Regular follow-up with identified students and families is a core part of the role.

2) Reintegration and Transition Support

The officer supports SwANs returning from prolonged absenteeism through gradual reintegration plans, which may include curtailed hours and supported transitions back to class routines. This includes providing emotional and behavioural support, engaging students meaningfully during periods outside the classroom, and facilitating positive connections with peers and trusted adults in school.

3) Case Support and Collaboration

Working alongside school leaders, teachers, counsellors, and external agencies, the officer contributes observations and updates to case discussions and intervention planning. Accurate and timely documentation of engagement notes, interventions, and student progress is expected.

4) Student Engagement and Programme Support

The officer supports and engages SwANs during school-based or vendor-led programmes, facilitates small-group activities and reflective sessions, and works with school personnel to create positive, motivating school experiences for students with higher needs.

5) Programme Planning and Implementation

In consultation with the Case Management Team, the officer designs and implements targeted programmes to address absenteeism and disengagement, facilitates these programmes independently, and regularly monitors and refines approaches based on effectiveness.

Working Hour:

This is a part-time role requiring approximately 2–3 days per week, with around 6 hours per visit (10am-4pm or 10:30am-4:30pm). Duties span both school hours and after-school hours, including home visits, programme facilitation, and student engagement activities.

Salary:

$50/h
