Counsellor - #1163547

A rehabilitation counsellor plays a key role in facilitating clients with mental health conditions reintegrate back to the community.

Key Responsibilities:

Provides a person-centred case management services to clients residing at the Psychiatric Rehabilitation Home, which include

• Conducting assessments

• Developing individualised care plans

• Provide supportive counselling

• Identify linkages to community resources (e.g. public housing applications, community partners, support group etc)

Assess suitability of referrals seeking admissions

Provide care coordination and case management for clients

Conduct psycho-education sessions for clients and caregivers

Conduct review on progress of each client

Establish transition and discharge planning with clients

Collaborate with Case Management Assistant, Occupational Therapist, Job Coach and Centre Manager to ensure continuity care of the client, provide vocational support and organise suitable programmes and activities for clients

Participate in multi-disciplinary meetings

Any other duties assigned

Qualifications and Experience:

Degree in Social Work, Psychology, Sociology, Counselling or related fields

Skills:

Accredited as a Registered Social Worker (RSW) with Singapore Association of Social Workers (SASW)

2-3 years of relevant experience

Excellent interpersonal and communication skills

Open to consider candidates without mental health experience

Possesses leadership qualities and the ability to guide and support team members
