Responsibilities:

• Conduct psychosocial assessments, individual casework, and group work sessions for clients with mental health conditions and psychosocial needs.

• Develop, implement, and monitor care plans and therapeutic interventions to support clients' recovery and well-being.

• Plan, coordinate, and evaluate community mental health programs and outreach initiatives.

• Build and maintain professional relationships with clients, caregivers, community partners, and multidisciplinary healthcare teams.

• Provide crisis intervention, risk assessment, and support for clients experiencing mental health challenges.

• Support staff development, facilitate case discussions, and promote evidence-based mental health practices.

• Identify service gaps and contribute to the development, implementation, and evaluation of mental health services and programs.

• Maintain accurate case documentation and ensure compliance with organizational and professional standards.

Requirements:

• Degree in Social Work, or a degree in a related field with a Graduate Diploma in Social Work from a recognised institution

• Minimum of 5 years of relevant social work experience preferred

• Experience in mental health case management, psychosocial assessment, crisis intervention, and recovery-oriented practice is preferred.

• Excellent written and verbal communication skills

• Strong analytical thinking and problem-solving abilities
