Senior Wellbeing Coordinator, Population Health & Integrated Care - #1162920

You will lead and mentor community wellness staff to deliver person‑centred care for residents with chronic and complex health and social needs across community and outreach settings. You will screen residents’ needs, coordinate, develop and implement holistic care plans with regular follow‑ups. You will also coach patients and caregivers in chronic disease self‑management, fall prevention, healthy lifestyles, telecare use and financial support. In addition, you will ensure timely referrals, accurate documentation, as well as active participation in quality improvement.

Requirements

• Degree or Diploma with at least 8 years’ relevant experience and demonstrated competency

• Prior experience in health, social or community care sectors, with knowledge of basic screening tools, vital signs measurement and simple functional/fall‑risk assessments

• Collaborative team player with strong verbal, written and interpersonal skills, and the ability to coach and mentor junior Wellbeing Coordinators
