Responsibilities:

• Review and dispense medications accurately according to prescriptions.

• Perform medication reconciliation and review patients' medication histories.

• Monitor patients for medication effectiveness, side effects, and drug interactions.

• Provide medication counselling and education to patients and caregivers.

• Participate in multidisciplinary ward rounds and collaborate with doctors, nurses, and allied health professionals.

• Recommend appropriate drug therapy and dosage adjustments based on patients' clinical conditions.

• Support discharge planning by ensuring patients understand their medications before leaving the hospital.

• Manage medication inventory and ensure compliance with medication safety standards.

• Document pharmaceutical interventions and maintain accurate patient records.

• Participate in quality improvement projects, audits, and medication safety initiatives.

• Supervise and train pharmacy technicians, pharmacy assistants, and pharmacy students when required.

• Ensure compliance with hospital policies, regulatory requirements, and professional standards.

Requirements:

• Bachelor's Degree in Pharmacy or equivalent qualification recognized in Singapore.

• Registered Pharmacist with the Singapore Pharmacy Council

• Good knowledge of pharmacotherapy, medication management, and pharmaceutical care.

• Ability to work independently as well as collaboratively within a multidisciplinary team.

• Proficient in pharmacy information systems and Microsoft Office applications.
