Job Description:

As a Head Community-Based Services, you will be responsible for the following duties:

• Provide strategic leadership and direction for community-based services, ensuring alignment with the organisation’s mission, vision, and Singapore’s community care priorities.

• Lead the development and execution of departmental strategies, service roadmaps, and continuous improvement initiatives to achieve positive and measurable outcomes for clients and caregivers.

• Oversee the planning, implementation, and delivery of community-based services, including home care, community programmes, caregiver support, and care navigation services, ensuring high-quality, person-centred, and outcomes-focused care.

• Establish, review, and uphold service standards, policies, governance frameworks, and operational procedures to ensure compliance with regulatory, funding, and quality requirements.

• Cultivate and maintain strategic partnerships with healthcare providers, social service agencies, government agencies, and community partners to promote integrated and coordinated care delivery.

• Strengthen referral networks, service integration efforts, and collaborative care models to enhance service accessibility, continuity of care, and the overall experience of clients and caregivers.

• Lead the planning and management of departmental budgets, ensuring prudent utilisation and accountability of organisational and public resources.

• Ensure adherence to all regulatory requirements, funding agreements, organisational policies, and sector standards, including those established by Agency for Integrated Care (AIC), Ministry of Social and Family Development (MSF), and relevant licensing authorities.

• Drive quality assurance, risk management, and service governance initiatives to maintain service excellence, operational effectiveness, and regulatory compliance across the department.

Requirements:

• Bachelor’s Degree or Graduate Diploma in Social Work, Nursing, Allied Health, Community Development, or a related discipline.

• At least 5 to 8 years of relevant experience in the community care or social service sector

• Minimum 3 years of leadership and people management experience

• Experience working across healthcare institutions, social service agencies, and government partners is advantageous.
