Senior Executive, Primary Care (Part-Time 21 hours / Contract till March 2027) - #1133750

You will implement initiatives related to community mental health and ensure the completion of tasks with a focus on quality and impact. By understanding the end-to-end processes, you will apply critical thinking to assess effectiveness and identify opportunities for improvement. Clear communication and proactive problem-solving are essential as you engage with patients, stakeholders, and cross-functional teams.

You will also support broader community health efforts, including place-based care initiatives, to meet the population health needs in the East.

Responsibilities:

• Collaborate with stakeholders to plan, draw up processes and gather feedback as part of project execution.

• Ensure cross-functional perspectives are considered, including resource impact on patient journeys.

• Facilitate projects, manage resources, and support stakeholder engagement to achieve outcomes

• Build and maintain strong networks with community healthcare and industry partners

• Manage data collation and apply analytics to support planning and development of care integration initiatives

• Monitor performance indicators and generate reports for funders and project teams

• Identify gaps and suggest improvements based on data insights

• Provide secretarial support to workgroups and ensure meetings are well-organised and outcome-focused

• Streamline workflows and ensure smooth project operations

• Engage stakeholders across units to align efforts with operational goals

• Track expenses and funding if any, for departmental expenditures and ensure implementations are within reasonable budget

• Any other duties assigned from time to time

Requirements:

• Bachelor’s Degree in any relevant discipline

• Minimum 2 years of working experience, preferably in healthcare or social service-related industry

• Good interpersonal & communication skills

• Adaptable to new changes and willing to learn

• Able to work independently with minimal supervision

• Proficient in Microsoft Office
