Job Title: OCCUPATIONAL THERAPIST
DEPARTMENT REHABILITATION
POSITION TITLE OCCUPATIONAL THERAPIST
RESPONSIBLE TO Director
BROAD FUNCTION:
Performs overall Occupational Therapy Assessment and Treatment. Maintains optimum standards of Occupational Therapy care and practice through assessment, problem identification, treatment planning, implementation, re-evaluation and discharge planning for client diverse presentations of physical and psychological conditions.
Engages in continual education (in-service, case discussions and presentations, workshops and conferences) on a regular basis.
Undertakes all aspects of clinical duties as an autonomous practitioner, including delivery of health promotional activities.
Initiates and develops projects to improve service delivery standards.
The Occupational Therapist is competent in managing basic and routine cases, and is able to provide coverage for colleagues as needed. He or she may be required to provide supervision to students on clinical attachment, and will undergo client management and documentation audits by senior staff on a periodic basis.
The candidate is required to be certified by the Allied Health Professions Council.
PRINCIPAL RESPONSIBILITIES:
Clinical
1. Responsible for a delegated caseload, managing clients presenting with a variety of basic and complex conditions.
2. Professionally accountable for all aspects of his or her work and training of support staff.
3. compliant with the requirements specified.
4. Maintains appropriate individual treatment records documented in the SOAP format.
5. Assesses clients and develops clinical diagnoses using Standardized and non standardized assessment test, outcome measures and clinical reasoning to ensure optimal treatment outcomes.
6. Plans, develop and implement Individual Care Plans effectively and independently. And in collaboration with nurses and allied health professional
7. prescribe appropriate equipment and assistive devices and provide training for safe and effective use during treatment sessions
8. Demonstrates sound knowledge of clinical risk assessments and proactively ensures a safe environment during treatment of clients.
9. Effectively and independently engages clients, family members/caregivers and other members of the care team in the development and implementation of care plans.
10. 10. provides holistic care for each client by ensuring appropriate and timely referrals to external agencies and community partners for psychosocial support and continuity of care after discussion with the multi-disciplinary teams
11. Conducts training for support staff as and when needed.
12. Provides supervision for therapy assistants/aides
13. Provides fieldwork education, guidance, supervision and mentorship to students on clinical attachment.
14. Works closely with occupational therapist colleagues from other clinical settings/ team, providing peer group support.
15. Work at all times within the scope of the Workplace Safety and Health Act of 2009 and the Home policy for reporting of accidents, incidents, hazards and risk management.
16. Maintain and protect the safety, confidentiality and dignity of the clients and families.
17. Maintain the principles of the Personal Data Protection Act of 2012 both within and outside the Home environment.
Professional / Staff Development:
1. Actively maintains own clinical competency for practice, through research and access to internal and external resources, forum and network as identified by self and the Director.
2. Promotes and implements evidence-based practice and standards
3. Provides appropriate clinical advice and assistance to junior therapists and therapy aides
4. Develops effective client and caregiver educational materials and training
5. To participate in the performance review/appraisal process as scheduled.
6. Support lor facilitate research initiative with in Occupational therapy service.
Other Activities
1. Work with the multi-disciplinary team in integrating the clients into the community
2. Attend relevant meetings and give necessary input.
3. Undertake responsibility and duties assigned by the Director from time to time.
Qualifications:
Diploma, Bachelor's degree in Occupational Therapy recognized by Allied Health Professions Council (AHPC) Singapore, or have passed the Qualifying Examinations recognized by AHPC.
Other Requirements:
Full or restricted registration with AHPC
Valid practicing certificate
Training and experiences:
Adequate administrative, organizing, writing and interpersonal skills to take on the above mentioned tasks, as well as the compassion to help the needy and the disadvantaged.
