Primary Objectives of Position
Implement and develop the CREST programme to deliver service in accordance to the intended programme objectives and care model.
Promote mental health and dementia literacy, early identification and link those at risk/diagnosed to the programme for continual support by by performing community outreach and community development work.
Adopt a case management approach to provide psychoeducation, psychosocial interventions, service linkage, care coordination and follow-up to clients, including caregiver support.
Job Responsibilities:
Plan and conduct outreach events in community to promote mental wellness, health awareness and information.
Engage community partners such as grassroots, to raise awareness, education and training on mental health and ensure smooth conduct of these events.
Conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
Establish and/or maintain network with stakeholders to build collaborative partnerships and facilitate care and support for clients and caregivers.
Carry out case assessments/re-assessments, provide information and referral, mental health information and emotional support to clients and caregivers, via home visits and/or phone-calls.
Facilitate communications with client/family/caregivers and/or partners to monitor and coordinate the care needs for clients.
Support in the planning and implementation of group work, support groups and community engagement initiatives that meet programme objectives.
Attend regular case-based learning platforms, including case discussions, and mental health/dementia training to fulfill competencies of programme.
Ensure and maintain accurate client records and programme data and documents, in accordance to programme requirement, stakeholders reporting and prevailing personal data protection security guidelines.
Collate and compile client statistics and data, and submit accurate and timely reports to stakeholders.
Support in programme development, monitor trends and issues relating to dementia/mental health services and needs of clients and their families.
Job Specifications
• Diploma in Social Work or relevant disciplines such as Psychology, Gerontology or other relevant fields in Social Science
• Candidates without experience are welcome to apply
• Candidates with at least 2 years of relevant experience working with persons with mental health needs and/or older adults will be advantageous
• Microsoft office
• Meticulous and possess good organising skills
• Strong team player with initiative
• Enthusiastic and passionate for community engagement
• Cheerful and empathetic with strong passion in mental health
• Good communication skills, interpersonal skills, customer service skills and language proficiency to connect with diverse populations, especially seniors
