Duties & Responsibilities:
Conduct comprehensive biopsychosocial assessments to understand residents’ social support, mental health needs, coping abilities, and environmental factors.
Develop individualized care plans in collaboration with residents, families, and multidisciplinary teams, with measurable goals and tailored interventions.
Provide supportive counselling to residents and their families, addressing emotional needs, concerns, grief, and adjustment issues.
Identify and coordinate internal and external resources to meet residents’ psychosocial, emotional, and practical needs.
Liaise with community partners, SSAs, hospitals, and government agencies to ensure appropriate support.
Participate actively in discharge planning meetings, assisting residents and families with transitions to community living, step-down care, or long-term arrangements.
Maintain accurate and timely documentation of assessments, care plans, progress notes, and case updates in compliance with regulatory and audit standards.
Provide guidance to families on coping strategies, available resources, advance care planning, and end-of-life considerations.
Maintain up-to-date knowledge of geriatric care, counselling practices, social service frameworks, and case management standards.
Support Home-wide programmes, initiatives, and community engagements.
Perform any other duties as assigned by the Social Work Supervisor or Head of Home.
Requirements:
Bachelor’s Degree in Psychology, Counselling, or related field.
Relevant experience in healthcare, eldercare, or geriatric social service settings preferred.
Proficient in psychosocial assessment tools, counselling techniques, and case management approaches.
Strong interpersonal skills with the ability to build rapport with elderly residents and their families.
Skilled in crisis intervention, problem-solving, and advocacy.
Familiarity with Singapore’s social service ecosystem, community resources, and support networks.
Commitment to ethical practice, confidentiality, cultural sensitivity, and diversity.
