Job Summary:
Care & Support Executive for an Active Ageing Centre (AAC) to assist in the day-to-day operations of the centre, manage inquiries, coordinate befriender volunteer engagement, and maintain records. The role involves outreach to seniors, understand their care needs and ensuring services are referred timely.
Principal Responsibilities and Duties:
Outreach & Engagement
Plan and lead outreach strategies to engage seniors in the community, with emphasis on identifying and connecting with socially isolated seniors.
Develop and implement targeted approaches (e.g., home visits, neighbourhood outreach, collaborations with grassroots partners) to raise awareness of centre programmes and services.
Track outreach activities and outcomes to ensure effectiveness and continual improvement.
Care & Support Services
Provide seniors and their caregivers with in-depth information and guidance on available programmes, services, and resources.
Conduct needs assessments and makes appropriate referrals to community partners, healthcare providers, and social service agencies.
Ensure follow-up support for seniors and caregivers, monitoring progress and addressing gaps in care.
Volunteers Management
Recruit, train, and manage a pool of befriender volunteers to support seniors through companionship and care.
Oversee volunteer engagement strategies, ensuring befrienders are equipped, motivated, and recognised for their contributions.
Match volunteers to seniors based on needs, strengths, and suitability, monitoring the sustainability of the befriending relationship.
Operations & Administration
Establish and maintain efficient systems for case recording, volunteer management, and reporting of outreach and care activities.
Monitor service delivery standards, ensuring compliance with organisational SOPs and funder requirements.
Contribute to data collection and outcome measurement to evaluate impact and support decision-making.
People Leadership
Leads AAC officers and volunteers. Provides support, motivates staff and volunteers, ensures well-being of team members at the ground level.
Essential Skills and Qualifications:
Diploma in any discipline, preferably with 2 - 3 years of relevant experience in eldercare or healthcare sector.
Preferably with experience working with elderly clients in a community setting, especially in healthcare or social service sectors.
Strong interpersonal and communication skills, the ability to work independently and as part of a team, and good organizational and time management skills.
A passion for working with seniors and a commitment to promoting active ageing.
