Established in 2006, THK Early Intervention for Children Services (THK EIC Services) is part of THKMC. We provide early intervention (EI) to children under 7 years old who have been diagnosed with developmental needs. Our centres cater to children with varying needs, providing specialised intervention for different levels of support requirements and EI needs.
Reports to the Assistant Senior Social Worker, Senior Social Worker, HOD (Social Work)
Main Duties and Responsibilities:
Case Management
Holds up to 50-60 cases of enrolled students
Facilitate parents' understanding of programme and needs of child.
Establish rapport, engage parents and conduct intake assessments using Bio-Psycho-Social assessment (BPS) of child's and family's needs, highlighting family's risks and strengths.
Conduct Routine-Based Conversations (RBC) during home visits and follow up sessions to support parents.
Evaluate and identify areas for follow up (e.g. referral to internal/external resources).
Ensuring reports and case recordings are up to date.
Trans-Disciplinary Collaboration
Participate actively in case discussions, case conferences, and team meetings with professionals such as Teachers, Psychologists, Occupational therapists, Speech and Language therapists, Physiotherapists.
Share social work perspectives and insights to support holistic intervention planning.
Implement intervention plans collaboratively with the trans-disciplinary team to address clients' developmental, behavioural, or social-emotional needs.
Social Work Administrative Matters:
Ensuring relevant documentations are updated in students' e file
Support and assist in audit matters such as periodically checking of documents and e-filing matters.
Conducting Means Testing and Deviation (if needed)
Collaborative Partnerships/Community Network
Demonstrate knowledge of available community resources
Engage and work collaboratively with various community agencies (e.g Family Service Centres, Social Services Offices or other agencies) to ensure holistic case management for the child and family.
Coordinate care plans and follow-up actions with internal and external partners to ensure integrated support for clients.
Programmes/ Workshops
Plan and facilitate programmes such as Parents' Orientation, Siblings' workshop, Parents support group and other programs.
Support in other Centre-based programmes if required
Qualifications
Minimum Degree/ Postgraduate Diploma in Social Work
1 to 3 years of experience working with families and children preferred, fresh graduates may apply
Other Information

Has patience and commitment in the education of and nurturing young children, empowering the families
