He/ she will support in case management through collection of bio-data of resident/family through interviews and to provide social information to the CARES and clinical team to facilitate in management of resident care.
Job Responsibilities
Gather social and financial background of the resident and/or the family prior the application of the financial assistance schemes.
Updating information in the overall referral and enquiries database and information system.
Ensure timely submission of financial assistance schemes for residents in the home such as Medifund, MFEC (Medical Fee Exemption Card, IDAPE (Interim Disability Assistance Programme for Elderly), PGDAS (Pioneer Generation Disability Assistance Scheme) and etc.
Single Point of Contact and liaison for residents, family and healthcare institutions to co-ordinate referrals and process applications of potential residents.
Ensure proper documentation of case files, referrals and other related documents for case management and audit purposesPerform data entry and update information regularly to ensure accuracy of database.
Perform administrative duties such as handling enquiries, referrals and admissions.
Facilitate transfer to another facility or other healthcare institute as well as discharge of residents.
Any other duties/ projects as assigned by the supervisor
Job Requirements
'O' Level or Diploma qualification
Service orientated with good interpersonal and communication skills
Dynamic team player with integrity and commitment
Proactive and self-motivated
