AM or PM Shift

Must be able to work at least 3-5 days a week

Job Description: 

• Provide direct patient care, including monitoring vital signs, administering medications, and assisting with daily activities.

• Collaborate with multidisciplinary teams to develop and implement individualized care plans.

• Conduct assessments and evaluations of patients' health status, documenting findings accurately.

• Educate patients and their families on health management, medication regimens, and lifestyle modifications.

• Ensure compliance with medical protocols, infection control measures, and safety standards.

• Respond promptly to emergencies and provide appropriate interventions.

• Maintain accurate and up-to-date medical records and reports.

• Participate in continuous professional development and training activities.

• Adhere to regulatory requirements and professional standards of nursing practice.

• Demonstrate empathy, compassion, and professionalism in all interactions with patients and colleagues.

Job Requirement :

• Must possess Degree / Diploma in Nursing

• Min 1-2 year of experience in Home-based Nursing, Nursing Homes, Acute hospital or community hospital
