As an Executive, Case Management, you will be responsible for the following duties:

• Coordinate care and connect clients who have or are at risk of mental health conditions and/or dementia with appropriate resources, including caregiver support.

• Develop and implement outreach initiatives targeting the community and key stakeholders—such as faith-based, corporate, and community organizations—to raise awareness and promote mental wellness.

• Develop and execute targeted preventive initiatives to enhance mental well-being in the community, particularly for at-risk individuals and clients.

• Conduct routine home visits to assess and monitor the well-being of elderly clients

• Partner with a range of external stakeholders to develop and implement initiatives that promote senior engagement and well-being.

• Any other ad-hoc duties as assigned

Requirements

• Possess a Diploma in Community Care & Social Services or equivalent
