Responsibilities:

• Provide comprehensive patient care management for elderly in the community, including assessments, physical examinations, care planning, follow-ups, and clinical procedures.

• Deliver patient and caregiver education, including health promotion, discharge teaching, and training on care techniques.

• Uphold infection control standards through practice, monitoring, and public education.

• Guide and support junior nursing staff and care team members in delivering quality care.

• Maintain professional development through ongoing training, supervision, and participation in learning activities.

Requirements: 

• Diploma or Degree in Nursing.

• Registered nurse with Singapore Nursing Board.

• Advance Diploma in Geriatrics, Medical-Surgical, Chronic Disease Management, Community Health is highly advantageous.

• At least 2 years' experience nursing experience in Singapore.

• Strong knowledge and understanding of community care practices.
