As a Social Work Assistant, you will be responsible for the following duties:

• Perform assessment in financial application cases.

• Assisting patients in national financial schemes application.

• Assist medical social worker in reviewing assessing cases.

• Work together with medical social worker in providing brief social assessment.

• Collecting and organizing information for case assessment.

• Perform home visits when necessary.

• Educating patients and family members.

• Act as mentors to new Social Work Assistant within the team.

• Participate in quality improvement projects when necessary.

• Any other duties within the team.

Requirements:

• Min. Diploma in Social Work or equivalent.
