As a Social Worker, Home Care, you will be responsible for the following duties:

• Manage a portfolio of cases by assessing needs and implementing interventions to support patients and their families.

• Link clients to appropriate community resources and services, including arranging home visits when required.

• Collaborate with doctors, nurses, and allied health professionals to provide holistic care.

• Contribute to the development and execution of departmental initiatives, programmes, and outreach activities.

• Participate in professional supervision and continuous training for skills development.

• Carry out additional duties as assigned by supervisors.

Requirements:

• Possess a Degree in Social Work/ Post Graduate Diploma in Social Work or equivalent.

• Registered with Singapore Association of Social Work.

• Experience in healthcare setting is advantageous.
