Responsibilities:
Conduct means tests, financial assessments, and intake screenings.
Build rapport and maintain professional relationships with clients and caregivers.
Assess case complexity, risks, and protective factors, and prepare intervention plans.
Develop and implement case histories, assessments, and care/case plans.
Lead admission and discharge processes in collaboration with families, the multidisciplinary team, and external stakeholders.
Support residents and families in accessing resources, including financial assistance.
Monitor and review care/case plans regularly to ensure suitability and effectiveness.
Participate in developing interdisciplinary care plans and highlight critical cases during meetings.
Maintain proper case documentation, reports, and communication logs.
Perform other duties as assigned by the supervisor.
Build and maintain collaborative relationships with ministries, social service partners, and community agencies.
Participate in outreach initiatives within the disability sector.
Ensure compliance with government and social service documentation requirements.
Cover Day Activity Centre Manager duties during periods of extended leave.
Manage collection of programme and transport fees.
Prepare periodic reviews and reports for statutory submissions and audits.
Handle social work reports, like Monthly Register, SW Monthly Report
Requirements
Bachelor’s degree in Social Work, Psychology, or related field.
Proven experience in social work, case management, or community services.
Strong knowledge of social service policies, procedures, and government requirements.
Experience conducting assessments, developing care plans, and implementing interventions.
Ability to build and maintain professional relationships with clients, caregivers, and stakeholders.
