Job Responsibilities:
Assist the Manager of the AAC and AAC Allied Health Team Lead to:
1. Propose and implement active ageing programmes that cover both physical and psycho-social wellness programmes that focuses on senior’s physical and psycho-social well-being covering the following:
Conduct group based Allied Health and Centre’s based activities and programmes. Recommend enhancements needed to better meet the needs of the seniors.
Identify and promote potential users of gym equipment.
Conduct initial assessment and 6 monthly re-assessment for the gym user.
Track and monitor gym users for their progression and follow-up and discharge plan, if any.
Guide the gym user on proper alignment during their gym session.
Conduct baseline physical assessments, manage regular screening sessions, analyze and recommend follow-ups for the various segment of seniors to Allied Health Team Lead, for concurrence.
Manage regular reviews to monitor the seniors’ compliance to exercise programmes, provide recommendations on follow-ups needed, to Allied Health Team Lead and follow through with the plans.
Maintain exercise and gym equipment including scheduling regular servicing and gym equipment cleaning supplies.
Implement new physical fitness programme and assist to publicise the programme.
Support community outreach and befriending of seniors. Conduct community screener assessments to achieve Centre’s service requirements.
Assist with vital signs monitoring
2. Centre’s Programme Co-ordination and Administration
Support the Centre Manager to develop and implement suitable active ageing programmes that would build the seniors’ self-help capacity, increase social and community participation.
Manage the registration of seniors participating in active ageing programmes including data collection entry.
Manage the collection of Allied Health related data for submission to Allied Health Team Lead. Analyse data outcome.
Prepare monthly reporting on targeted outcomes
Maintain documentation for audit purpose
Ensure compliance to Finance and Human resources policies
Perform any other administrative duties as assigned by Centre Manager.
3. Volunteers Engagement
Assist the Volunteer Management Executive to engage volunteers to conduct centre-based activities such as physical exercise and active ageing programmes, oversee gym exercises, etc.
Encourage seniors and empower them to step up and volunteer for physical and psycho-social well-being programmes.
4. Team Contribution
Share best practices with Wellness Coaches.
Assist Allied Health Team Lead in all Allied Health related matters.
Work with and support Centre team to ensure Centre is able to achieve its service requirements and MOH’s KPIs.
Job Requirements:
Minimum Diploma holder – any relevant diploma in healthcare, sports or social services.
Able to lead exercises independently
Good interpersonal communications skills
Passion for community work and keen to work with the elderly
Is a team player
Ability to work fairly independently
Self-Motivated
Good in Microsoft Word, Excel and Powerpoint.
Comfortable with using tech-enable devices
Able to influence, guide and empower seniors to lead an active lifestyle
Support and encourage seniors’ volunteerism
