Job Description
To provide holistic care and support to the physical and psychosocial well-being of clients in the DAC.
Plan and execute daily activities for clients based on AWWA DAC’s programme, according to Person Centred Active Support.
Provide assistance in activities of daily living (e.g. feeding, toileting, transfers etc.) as required by clients.
Effectively communicate with Centre Supervisor and team members of AWWA DAC.
Work within a multi-disciplinary team and family/caregivers to assess, identify, implement and execute services and activities to meet the needs of the respective clients through an individualised care support plan(ICSP).
Retrieve and maintain accurate programme records of all clients in AWWA DAC in a timely manner.
Attend meetings as and when required by the programme.
Responsible for the upkeep and maintenance of all programme resources.
Assist in ad-hoc projects/committees as and when required by the Centre Supervisor.
Requirements
At least a Diploma in a relevant discipline, such as Therapy, Sports and Exercise etc.
Minimum 2 years of experience in the Healthcare sector working with Disability clients.
Passion to serve the underprivileged.
Can work independently and in a team
Excellent interpersonal and communication skills
