Job Scope:
Reporting to the Centre Manager, the Community Care Officer works closely with the Centre in-charge to handle the daily operations of the Active Aging Centre (AAC) and engage seniors living nearby in the community to participate in activities that support active lifestyles and promote their well-being.
Key Responsibilities:
Outreach and Engagement
Conduct regular outreach to seniors in identified residential address to promote and advertise the activities to seniors.
Support activities and outings organised for the seniors.
Assist in collecting data to meet reporting requirements and collating ground feedback for programme evaluation and review purposes.
Develop and implement outreach plan for service boundary, monthly activities calendar and an annual calendar plan of health and social activities for seniors.
Befriending and Buddying
Review and carry out regular home visits for socially isolated seniors needing the services to monitor their health and well-being and adhere to service requirement.
Manage referral system to receive and update status of cases.
Care Information and Referrals
Serve as a point of contact for seniors for various schemes and grants for seniors and their caregivers, to answer ground queries, as well as provide effective and efficient service referrals to the relevant agencies or departments for follow-ups.
Volunteer Management and Community Partnerships
Drive and contribute to ASH’s volunteer management framework, by actively recruiting, orientating, training, retaining, deploying, engaging and appreciating volunteers in alignment with the assigned roles, mission and objectives of the AAC.
Represent ASH in community partnership networking sessions, such as Communities of Care, multi-agency case discussions and meetings.
Day-to-Day Centre Operations
Handle walk-in enquiries and phone calls.
Maintain proper records, documentation on the calls, engagement/activities and referral of the engaged seniors.
Any other duties as assigned by Reporting Officer (Centre Manager).
Job Requirements:
Minimum Diploma in any discipline from an accredited Polytechnic. Preferably in the field of Gerontology, Social / Healthcare Services, Community Development, Psychology, Social Work, Counselling.
At least 1 year of experience in front-facing, direct operational work in eldercare or social services. Candidates with prior volunteering experiences with the elderly are preferred.
Proficient in IT and Microsoft Office applications.
Good interpersonal skills: resourceful, proactive, responsible and client-centric. Passionate in serving seniors and volunteers.
Good team player yet can work independently, ability to multi-task and meet deadlines in a fast-paced environment, have good planning and organisational skills.
