We are seeking a highly motivated and nurturing Senior Social Worker to join the Community Care team. Reporting to the Function Lead, you will be responsible for patient case management, administration of subsidies and welfare programmes, and ensuring compliance with MOH & NKF policies. You will also play a key role in providing counselling, stakeholder collaboration, and process improvement to enhance patient care and operational efficiency. Your responsibilities are as follows:
1. Patient Case Management & Social Work Practice
Manage ESRD patients in dialysis centres or home dialysis (HD/PD) through client engagement, conducting interviews, data collection, case assessment (BPSS, risk & needs assessment, and other relevant assessment tools), case conceptualisation, case planning, and intervention plans for best patient outcomes.
Provide supportive counselling through home, dialysis centre, or hospital visits.
Work collaboratively with healthcare professionals to ensure holistic patient care.
2. Subsidy & Welfare Programme Administration
Oversee the application and approval of NKF subsidies and welfare programmes.
Ensure accuracy and alignment of means-testing results across internal systems.
Assist the Assistant Manager in evaluating and approving social assistance requests.
3. Service Management & Compliance
Ensure compliance with MOH and NKF policies in subsidy administration and patient welfare programs.
Maintain accurate records and align financial assistance processes across systems.
Support internal and external audits through data extraction, reporting, and compliance reviews.
4. Supervision & Professional Development
Provide supervision and training to Social Workers, upholding ethical and clinical best practices.
Maintain high standards in case management through knowledge sharing and mentorship.
5. Stakeholder Engagement & Process Improvement
Collaborate with internal departments and external partners on program-related matters.
Identify opportunities to streamline work processes and propose modifications to enhance accountability and efficiency.
Prepare statistical reports for ongoing performance tracking and bulk approvals.
Qualifications
BA majoring in Social Work/Graduate Diploma in Social Work.
Registered Social Worker (RSW) with the Singapore Association of Social Workers.
Experience
Experience in a social service agency, with at least 5 years in direct social work practice.
Experience in administration and accounting-related work is a plus.

Stakeholder Collaboration – Strong ability to engage with internal teams, government agencies, and healthcare partners.
