Wellness Coach (North) - #1102238

• Identify, promote and implement activities and wellness programs at the community dialysis center.

• Manage screenings, physical assessments, data compilation, analysis, and recommendations for follow-ups for dialysis patients.

• Conduct regular reviews to monitor patients' compliance and progression in the exercise program and ensure care plans are followed through.

• Maintain exercise and program equipment and ensures regular disinfection and servicing.

• Manage patients’ attendance to ensure accurate and timely documentation to maintain consistent participation.

• Collaborate with Clinical Services Team to address any barriers to participation and ensures patients receive the necessary support to continue their wellness journey.

• Assist in organizing volunteer events and outings to empower seniors and encourage engagement.

• Any other matters as required.

Job Requirements

• Minimum Diploma in Health Sciences (Health Management) or other relevant courses (e.g., sports sciences, therapy aide, community care associate).

• Relevant working experience in a healthcare setting is advantageous.

• Strong interpersonal skills and proficient in Microsoft Office.

• Positive and committed attitude with a strong desire to serve the community.

• Highly organized, detail-oriented, and multitasking ability.
