Job Description
Responsibilities:
- Understand and provide clients with information on available health and social benefits, and match clients accordingly
- Monitor clients' care plans by coordinating maintenance and/or rehabilitation programmes and support them in the enrolment of health and community programs
- Involved in the planning and implementation of activities organized for the clients in the community
Requirements:
- Diploma in Community Care, Social Services or any other relevant discipline
- Candidates with no prior experience are welcomed to apply
- Passionate in elderly care and willing to work in a flexible schedule when required
