As a Care Coordinator (Healthcare), you will be responsible for the following duties:

• Work with a team of doctors and nurses to take care of a group of patients with chronic conditions.

• Screen patients for relevant health issues.

• Guide patients to understand the importance of annual eye, foot and health screenings.

• Assist and educate patients about self-care, performing vision testing and basic assessments.

• Coordinate the care for patients with the healthcare team.

• Any other ad hoc duties as assigned.

Requirements

• Possess a Diploma in Health Management & Promotion or equivalent.
