Area of Accountability

1) In charge of nursing and custodial care to all SCC/AAC Clients and provide necessary Nursing care support to AAC Clients.

• Perform assessment to new clients and reassessment to all Day care clients according to service requirement.
• Develop, review and update ICP with multiple disciplinary approach and Person- Centred Care approach.
• Communicate with team members and clients/family for any clinical concerns of Day-care clients.
• Ensure proper nursing documentation according to service requirement.
• Consult and coordinate with health care team to assess, plan, implement and evaluate client’s condition.
• Provide health promotion, counselling to clients and families.
• Oversee nursing care such as medication administration, wound dressing, ADL assistance, hygiene care, nutritional care, and other technical skills as required.
• Work together with Dementia Practitioner (OT and Centre Manager) for behaviour issues addressed from Dementia Day Care clients and any other Day Care Clients.
• Share and train care staff and ensure care staff deliver clinical care according to clients’ care needs as part of PCC approach.

2) Compliance with approved standards of nursing practices and ensure team competency in SCC/AAC.

• Assist Centre Manager in Overseeing and managing the performances of the nursing team / care staff to ensure desired level of Clinical nursing work competency.
• Assist Centre Manager in coordinating, implementing, and monitoring clinical and service quality performance indicators for continuous improvement plan in SCC/AAC.
• Perform CPR, AED and first aid to clients and plays an active role in emergency situations.
• Assist and support delivery of health promotion activities in the Centre.
• Promoting PCC and MDT approach as service required.

3) Maintenance of nursing Clinical equipment and inventory of Nursing clinical items according to service requirement and company rule

• Monitor nursing clinical equipment function (such as, Hypo count machine, thermometer, parameter machines) and request for servicing accordingly.
• Request replacement or new order for nursing clinical equipment when necessarily.
• Inventory checking for Nursing clinical items and request ordering according to service requirements and organisation rules (such as, PPE, thermometer cover, wound care items).

4) Quality Assurance and others

• Participate in quality procedures and guidelines and policy reviews when required.
• Ensure compliance to the Company’s quality procedures, guidelines, and policy. Monitor and report non-compliance and implement necessary changes resulting from quality assurance findings in consultation with Physiotherapist or Occupational Therapist.
• Undertake responsibility and duties as assigned by Centre Manager/Nurse Manager.

Any professional License required

• SNB registered

Any other knowledge required

• Good communication & interpersonal skills
• Basic Computer skills for activities, documentation, and recording.
• At least 2 years’ experience in community care setting would be an advantage.
• Having Dementia Care training would be an advantage.
