Job Responsibilities
Coordination and Collaboration:
Facilitate collaboration among Senior Care Centre, Active Ageing Centre, Home Personal Care+, and Home Therapy providers to establish roles, responsibilities, processes and workflows, ensuring smooth implementation and delivery of ICCP services and achieving ICCP KPIs for the specified sub-region.
Lead ICCP partners' leadership meetings to set strategic directions, review performance and address service gaps, and evaluate resource opportunities.
Organize ICCP partners' operational meetings to plan and manage the effective delivery of the four core eldercare services in the sub-region.
Reporting & Administrative:
Oversee timely submission of progress reports and claims to MOH and AIC.
Act as the main point of contact for ICCP-related administrative matters.
Job Requirements
Minimum Bachelor’s Degree in healthcare management, social work, gerontology, or related field.
Minimum 4 years of experience in healthcare coordination, community care, or a related field, with a proven track record of managing partnerships.
Exceptional leadership, communication, and interpersonal skills, coupled with the ability to plan, execute, and collaborate effectively with diverse stakeholders
