[Psychiatric Nurse keen to move out] Mental Health Care Coordinator (Up 5K) #HJC - #1093801

Responsibilities and Duties:

- The Care Coordinator is to conduct home visits to provide psycho education, support and linkages cum care-coordination of services to clients and caregivers.

- Conduct assessment of patients for medical, psychological, social, environmental and vocational needs.

- He/She has to be a team player to collaborate with a group of Allied Health Professionals, doctor and nurses.

- Provide psychological input and expertise during multi-disciplinary meetings.

- Provide trainings to team / caregivers as necessary.

- To conduct Mental Health support group for patients and /or caregivers.

- To carry out projects and duties assigned by supervisor or Head of Department.

Job Requirements:

- Registered Nurse with Singapore Nursing Board.

- Possess a RMN/Adv Dip in Mental Health or other relevant qualifications (e.g. social work, counseling, psychology).

- Experienced in psychiatric setting for at least 3 to 5 years will be advantage.

- Has experience using psychotherapy with mental health patients.

- Passion to work in the community.

- Possess strong organizational, interpersonal, communication and writing skills.

- Good computer literacy skills – MS Office etc.

- Familiar with healthcare environment.

- Ability to multi-task, work independently and collaborate as a team.

- Passion in managing people with mental health disorders.
