Sr Executive / Assistant Manager, Population Health & Integrated Care (Contract) - #1163730

You will collaborate with clinics, hospitals, specialty care providers, and community resources within the Primary Care Network (PCN) to support patients enrolled in the PCN/Healthier SG scheme. These include monitoring the patients’ progress and ensuring timely delivery of holistic care by liaising with various internal and external stakeholders. In addition, you will work closely with the PCN team to manage challenges faced during the care delivery process as well as be a key participant in quality improvement and primary care partnership initiatives.

Requirements

• Degree with at least 2 years of working experience

• An effective communicator across all levels in an organisation

• Good command of written and verbal English

• Able to work independently as well as in a team

• Meticulous and self-motivated team player who is able to multi-task, work under pressure and within tight timelines

• Proficient in Microsoft Office, especially advanced Excel (VBA/macros), with strong analytical and data visualisation tool/database development skills.

• This position covers maternity leave on a 1-year contract.
