Role & Responsibilities
The Executive – Community Care plays a vital role in supporting the delivery of comprehensive active ageing services to seniors in the community. This position combines broad operational support (70%) with specialized functional expertise (30%) aligned with the AIC Community Care Skills Standards Framework (CCSSF) for Active Ageing Centres.
Each Executive – Community Care will be assigned a specialist focus area based on organizational needs and individual strengths, while maintaining flexibility to support centre-wide operations and team members during high-demand periods.
General Operational Duties (70%)
Program Support and Implementation
◦ Assist in planning, implementing, and evaluating active ageing programs
◦ Support on-site and satellite activities for seniors
◦ Participate in outreach events and community engagement initiatives
◦ Collaborate with community partners to organize programs and events
◦ Conduct door-to-door outreach with Community Screener assessments
Administrative Support
◦ Maintain documentation for audits and compliance requirements
◦ Assist in data collection and reporting for MOH and AIC submissions
◦ Support general office administration including attendance tracking and record-keeping
◦ Contribute to achieving service requirements and meeting MOH KPIs
◦ Ensure compliance with Standard Operating Procedures (SOPs) at all times
◦ Assist with audit requirements and documentation
Centre Operations
◦ Answer phone calls and manage walk-in inquiries professionally
◦ Assist with procurement and inventory management for the centre
◦ Support facilities management and maintenance coordination
◦ Help coordinate centre-based activities and events
Cross-functional Support
◦ Provide backup support to colleagues during leave periods or high-demand periods
◦ Assist in cross-departmental projects and initiatives
◦ Participate in team meetings and contribute to centre-wide improvement efforts
◦ Perform other duties as assigned by the Centre Manager
Specialist Focus Area (30%)
Each Executive will be assigned one of the following specialist areas by the Centre Manager, based on organizational needs and individual strengths:
Case Management Specialist (Information and Referral - I&R)
◦ Perform needs assessments for community-dwelling seniors, including Community Screener, EBAS (for depression) and AMT
◦ Review care plans with relevant stakeholders
◦ Facilitate timely and cost-appropriate service referrals and provision to seniors
◦ Coordinate care with MWS services as well as external providers
◦ Participate in case management discussions within MWS and with external service providers
◦ Build and maintain comprehensive resource maps of services and touchpoints for seniors
◦ Maintain documentation for Register of Information and Referrals, Client Satisfaction Surveys, and CST documentation
Volunteer Management Specialist
◦ Recruit, onboard, and develop volunteers through targeted training programs
◦ Assign volunteers to appropriate roles matching their skills and interests
◦ Manage the Volunteer Management System (VMS) for registration and attendance tracking
◦ Engage volunteers from partner churches, community groups, schools, and corporate partners
◦ Coordinate befriending services for socially isolated seniors
◦ Monitor effectiveness of befriending through regular reports and feedback surveys
◦ Plan and coordinate outreach programs to identify and support isolated seniors
◦ Maintain recognition programs to motivate and retain volunteers
Wellness Program Specialist
◦ Manage and promote the use of gym equipment by seniors under guidance of MWS Allied Health Department
◦ Perform baseline physical assessments and regular reviews to monitor seniors' compliance with exercise programs
◦ Conduct or coordinate group exercise sessions
◦ Conduct wellness screening sessions for seniors
◦ Oversee maintenance of exercise and gym equipment, scheduling regular servicing and cleaning
◦ Track participation and outcomes of wellness programs
◦ Assist Volunteer Management Executive to engage exercise volunteers for centre programs
◦ Maintain registration of seniors participating in exercise programs and prepare data for submissions
Qualifications, Competencies & Experience
• Diploma or Degree, preferably in Community Services, Social Work, Healthcare, Psychology, Counselling, Business Administration, Sports Therapy, or related fields
• 1-2 years' experience in a relevant field; experience in social service sector will be an advantage
• Fresh graduates with passion for serving seniors are welcome to apply
• Passionate to work and interact with seniors in the community
• Strong interpersonal skills and ability to interact effectively with seniors, families, and diverse stakeholders
• Excellent communication and facilitation skills
• Good documentation and record-keeping skills
• Ability to multi-task and work effectively in teams
• Proficient in Microsoft Office applications
• Ability to work on weekends when required (time off-in-lieu will be provided)
• Willing to travel to different AAC locations when necessary
• Comfortable with using tech-enabled devices and digital platforms
