As a Care Coordinator (Primary Care Settings / West), you will be responsible for the following duties:-

• Collaborate with the multidisciplinary care team to deliver holistic and patient-centred care.

• Empower patients and caregivers to take ownership of their health through the use of technology and digital health tools.

• Promote awareness and accessibility of community and healthcare resources to support positive health outcomes.

• Build and maintain strong relationships with internal stakeholders, patients, and caregivers.

• Support preventive health initiatives by improving screening and vaccination uptake.

• Gather and document relevant social determinants of health to support care planning and interventions.

• Assist in coordinating and delivering patient care activities using IT systems for tracking, documentation, and monitoring clinical indicators.

• Perform basic health parameter measurements and support preventive care programmes.

• Provide basic health education on lifestyle management, physical and social activities, and self-care practices.

• Guide patients on the use of home health monitoring devices such as blood pressure monitors, glucometers, and weighing scales.

• Advise patients on recommended vaccination requirements and national health initiatives.

• Any other ad hoc duties as assigned.

Requirements:

• Possess a Diploma in Health Sciences (Health Management) or other related field.
