(Social Service) 7 Months Contract Healthare Executive / Counsellor (Up 3.5K) - #1160798

This incumbent will be required to support the Inpatient Clinical Operations Team to facilitate seamless transition of patients to the Hospital through proficient social screening and assessment of referrals. Ensuring that the bed allocation and bed management are able to facilitate expedient transfer of patients and optimal utilisation of overall bed capacity.

Job Responsibilities:

· Perform social and financial assessment of Inpatient referrals including financial counselling to patients and their families.

· Provide the necessary recommendations in the assessments.

· Assist with bed allocation and bed management, when necessary

· Participate in developing and implementing quality improvement initiatives to streamline department operations, to enhance patient outcomes and satisfaction.

· Perform other duties as assigned

Job Requirements:

· Degree in Psychology / Social Work or equivalent, preferably with 3-4 years’ social work experience.

· Have an interest in working on eldercare issues in a hospital setting.

· Is a good team player.

· Able to handle administrative role proficiently.
