Singapore Christian Home is looking for a compassionate Junior Medical Social Worker / Case Manager to support our residents and their families. This is an excellent opportunity for fresh graduates or early-career professionals who want hands-on experience, mentorship, and career growth in a mission-driven, community-focused environment.
What You’ll Do
Assess residents’ and families’ emotional, social, financial, and support needs.
Support smooth admission and discharge processes.
Conduct home visits, family conferences, and follow-ups.
Coordinate volunteer programs and community activities.
Collaborate with multi-disciplinary teams to deliver person-centred care.
Facilitate timely service referrals and assist with service quality initiatives.
Requirements
Degree in Social Work / Medical Social Work or related field.
Strong interpersonal and communication skills.
Passion for serving seniors and vulnerable groups.
Ability to work independently and in a team.
Willingness to work occasional Saturdays.
Able to speak Chinese to communicate with Chinese residents and their family members.
