Responsibilities:
Execute home-based nursing services and perform caregiver training.
Conduct comprehensive assessments of client, family, or caregiver situations to identify both resources and needs.
Design and review customized nursing care strategies, considering the client's preferences and requirements.
Ensure clear communication of these strategies to the patient and their family.
Execute and reassess the effectiveness of nursing interventions, such as care for wounds, management of medications, and continence support, with a constant commitment to safety and hygiene standards.
Organizing patient appointments efficiently, and utilizing scheduling tools to meet client requirements.
Foster supportive relationships with clients and their support networks.
Collaborate with a comprehensive healthcare team to ensure seamless care continuity and transitions.
Report any issues related to patient care or interactions with patients and their support networks to supervisory staff.
Prepare for patient discharge and facilitate referrals to ensure continuous care.
Guide and oversee the junior nursing staff and students and ensuring educational objectives are met.
Provide emergency nursing services outside of regular hours as needed.
Ensure precise and prompt record-keeping of patient care activities.
Handle financial transactions accurately, verifying patient eligibility for subsidies, issuing receipts, and managing funds appropriately.
Assist patients and families in accessing financial aid programs for healthcare.
Adhere to policies ensuring the confidentiality and security of client information.
Lead conversations with clients and their families, ensuring thorough documentation.
Support for grief and bereavement, including follow-up care for families and caregivers.
Contribute to quality assurance efforts to uphold and enhance care standards.
Support leadership in the development and implementation of procedural improvements with the services.
Engage in organizational committee or workgroup tasks as assigned by nursing leadership.
Any other ad hoc duties as assigned.
Requirements:
Bachelor’s Degree in Nursing.
A Registered Nurse with Singapore Nursing Board.
With 2-4 years working experience in home nursing/ community industry
