Intake, Admission and Discharge
Handle day-to-day general enquiries about the Home and DAC’s services and admission procedures
Support the coordination of referrals and admissions, including arranging intake assessments, liaising with external agencies and internal teams
Assist with administrative and logistical matters related to client/resident needs, from pre-admission to discharge, which includes (but not limited to) means-testing, financial resourcing, transport arrangements, etc.
Liaise with clients/residents and their families to ensure a smooth transition in and out of the service
Casework and Counselling
Support in basic case intake, assessments, planning and intervention as needed, in consultation with the supervisor/manager
Engage clients/residents and their families in a therapeutic manner to build rapport and collaborate on client care
Provide accurate and professional communication to clients/residents and their families about policies, procedures, and services
Work closely with the multi-disciplinary team members to support holistic, person-centered care for clients and residents, including (but not limited to) taking part in meetings and developing and setting Individual Care Support Plans (ICSP) for clients and residents
Liaise and coordinate with external agencies (e.g., schools, hospitals, community agencies) on issues related to client/resident’s care
Maintain timely and accurate documentation of case notes in compliance with PDPA and organisational policies
Programmes and Group Work
Assist with planning, coordination and execution of programmes for caregiver support and family bonding
Support pre- and post-programme evaluation through client and caregiver engagement, data-gathering and analysis
Be flexible to work occasional weekends to support such family and agency events
Administrative Functions and Governance
Maintain good housekeeping, confidentiality, privacy, and safety standards.
Manage administrative tasks such as scheduling appointments, coordinating meetings, muster reporting, and maintaining client databases
Prepare reports and referrals for review by the supervisor and management
Support the service’s adherence to Key Performance Indicators as set out by MSF and SG Enable, such as referral turnaround timelines
Conduct, collate and analysis data from the annual client and caregiver surveys, as required by MSF
Support the data collection for other audits and reports as needed
Assist with logistics and operational support when required
Others
Participate in team meetings, supervision, and relevant training for professional development and competency
To perform any other duties serving the aims of AWWA Adult Disability, as required by the supervisor, Centre Manager/Head of Home, and Director/Management
Job Specifications
Minimum Diploma in Social Work, Psychology, or related fields.
Experience in the social services sector, especially disability or residential settings, is beneficial but not required – candidates with relevant skills and passion are welcome to apply.
Knowledge of Mandarin / Malay / Chinese Dialects to communicate with clients and caregivers will be an advantage.
Has curiosity/passion and empathy to work with clients with disabilities and their caregivers
Well-organised and detail-oriented
Collaborative and reflective, with good communication and interpersonal skills
Takes initiative and able to work well independently, and as a team player
