Overview
As an Assistant Care Coordinator, you will play a key role in supporting patients, caregivers, and healthcare providers to ensure safe, effective, and compassionate care. This role involves coordinating care activities both at home and through telehealth, ensuring continuity and quality of service.

Responsibilities

• Conduct pre-admission screening for patients and caregivers.

• Support nurses in providing holistic care, including monitoring patient condition and progress.

• Identify and report care gaps, issues, or abnormal findings to the clinical team.

• Communicate regularly with patients and caregivers, providing updates and handling enquiries.

• Participate in interdisciplinary team meetings and contribute to care planning.

• Maintain accurate documentation of patient care and progress.

• Coordinate with community partners and healthcare providers for continuity of care and referrals.

• Assist in developing care plans based on patient needs.

• Monitor patients’ vital signs and escalate red flags to nurses/physicians.

• Provide basic patient education, lifestyle coaching, and health advice (e.g. diet, exercise, nutrition).

• Ensure adherence to telehealth monitoring protocols.

• Ensure availability of equipment and supplies, coordinating with vendors as required.

• Participate in quality improvement, risk management, and departmental projects.

• Carry out other ad-hoc duties as assigned.

Requirements

• Diploma in a health-related discipline preferred.

• Good computer literacy.

• Service-oriented, proactive, and adaptable in a fast-paced environment.
