Case Management
Assist in assessing residents’ and families’ needs, and recommend suitable emotional, social, financial, and support services
Support all admission and discharge procedures
Maintain accurate resident and referral progress records
Assist in evaluating referrals, conducting means tests, case management, and liaising with residents and families
Conduct regular follow-ups to ensure continuity of care
Carry out home visits and interventions such as family conferences, case conferences, and project care
Encourage and engage family involvement and visitation
Coordinate volunteer activities and programmes
Facilitate timely and appropriate service referrals for seniors
Support projects and programmes as required
Attend relevant training and medical social work supervision
Service Operations
Work closely with the multi-disciplinary team to deliver person-centred assessment, care planning, and discharge processes
Collaborate with residents and families to improve quality of life, independence, and social integration
Assist multi-disciplinary teams in service quality and improvement initiatives
Collate and analyse trends and needs of vulnerable groups
Others
Prepare weekly, monthly, and ad-hoc management reports
Perform other duties as assigned by the Manager, Medical Social Work or Executive Director
Working Hours
5-day work week, Monday to Friday, 8.30am to 6.00pm
Some Saturdays required for events involving residents
