As a Care Consultant, you will be responsible for the following duties:

• Reviewing and studying patients medical records / medical history

• Assess or interviewing patients to identify red flags that require intervention

• Perform appropriate test run and assessment

• Provide and recommend care plan by working with multidisciplinary teams

• Educating patients or caregiver to ensure optimal care

• Referring patients to appropriate healthcare providers when necessary

• Participate in continuous improvement process

Requirements:

• Min. Diploma in Nursing or relevant qualification

• Min. 3years of experience in bedside nursing care will be an advantage
