As a General Nurse (Home Team), you will provide safe and high standards of care to patients in their own home setting, ensuring a seamless continuity of care after their discharge from the hospital. An ideal General Nurse will be one who has the passion for inpatient and community nursing, and can thrive in a dynamic environment. Reporting to the Home Team Associate Director and Nurse Manager, the General Nurse’s roles and responsibilities include:
Role & Responsibilities:
Execute basic to complex nursing procedures in the patient’s home setting, under the Hospital at Home programme with restructured hospitals, offsite location and medical centres. This includes adult and paediatric population.
Independently perform nursing procedures such as medication administration via intravenous lines (e.g., CVC, PICC), withdrawing of blood samples, IV cannulation, physical assessment, vital signs monitoring including escalating abnormal readings, ECG monitoring, pain management, wound dressing, vaccinations, blood glucose monitoring, etc.
Independently conduct patient assessment, provide appropriate nursing intervention and evaluate outcome of intervention, including escalating required situations to doctors
Effectively communicate patient’s condition to internal and external healthcare team and provide accurate and timely updates via appropriate channels (including documentation on Client Management System)
Actively participate in handover meetings with partner hospitals to ensure seamless continuity of care
Provide caregiver training (CGT) for patients and family members as required, to ensure patients and/ or family are able to provide adequate care upon discharge from hospital
Ensure safe and high standards of care provision to meet clients' expectations
Perform any other duties which may be assigned.
Requirements:
Valid SNB registration as a Registered Nurse
BCLS + AED certification preferred
Hospital experience preferred; fresh graduates will be considered and adequate training will be provided
Ability to work under high pressure and manage situations effectively and independently
Team player with good initiative
Strong client and service oriented posture
5-day work week
Required to work shift hours and weekends. Shift hours are AM: 0700 to 1600 hours, PM: 1400 to 2300 hours, ND: 1800 to 0000 hours
No overnight shift required
Willing to travel islandwide, comfortable visiting patients home and carrying out nursing procedures in a home setting
